Physio Môn – Privacy Policy

Physio Môn respect your privacy and are committed to protecting your personal information, which we call “personal data”. This privacy notice will tell you how we look after your personal data and about your privacy rights. It supplements any other notices and is not intended to override them. We have tried to be brief and clear and we are happy to provide any additional information or explanation.

It is important that the personal data we hold about you is accurate and current. Please keep us informed of any changes.

The information we collect

We ask all new patients to complete a consent form, this allows us to collect information about you so that we can generate a patient record for the practice. The form requests the following information:

Name
Address
Date of birth
Telephone number
Email address
Your GP and their address
Any current medication you are taking
Health Insurance Provider details (if applicable)

This patient record will also contain any medical documents that you have provided us, physiotherapy treatment notes and forms you have completed and are all stored securely. We will never pass on your details to a third party without your consent.

Contacting you

We use the information you give us initially, either through the website (www.physiomon.com) or via phone or email to follow up on enquiries, to send you general information about it and our services, to ask for feedback, reviews or testimonials, or to deal with complaints. We may also contact you to ask if you would like to voluntarily complete a customer satisfaction survey.

We will also use your details to send you appointment reminders via text message when a mobile number is supplied. By providing your data and/or information, or by using our website or other online or digital platforms, you consent to the use of your data and information as described or referred to in this privacy notice.

Consent

In some instances we may have to write to your GP, Consultant, Health Care Provider, or solicitor. We will not send any information unless you are happy for us to do so or unless requested legally.

How long we keep your data

Your records are kept securely for 8 years, which is the legal data retention period for physiotherapy notes throughout the UK in accordance to Data Retention Policy. After this time your notes will be securely destroyed (unless you were under the age of 18 when you were seen at Physio Môn, in which case the retention period is until your 25th birthday).

In terms of personal data we use for marketing, we will keep this data for as long as we are able to market to you and if you withdraw your consent or opt-out of marketing communications, we will keep your contact details only to ensure that we do not contact you again for marketing purposes.

The right to erasure

You have the right to request erasure of personal information.

You have the right to request erasure from our email list, or you can click on the ‘unsubscribe’ link that appears in all emails we send. We will only use your personal information to send you marketing or educational material if you have given us your explicit permission.

We will consider all requests in conjunction with our legal obligation to retain information relating to your health care provided by us, as well as data protection law which clearly states when the right to erasure does not apply. Normally, this means we will not erase any information, unless it was not required for legal reasons

Right to access your information

You have the right to view, amend or delete personal information we hold about you. If you would like a copy of some or all of your personal information please write to Esther Cadogan, Physio Môn, Parc Mount, Glanhwfa Road, Llangefni, Anglesey LL77 7EY. You will not be charged for this process and you are entitled by law to a response within 40 days, however we would endeavour to respond sooner where possible.

Our cookies policy

The Physio Môn website makes limited use of cookies. Cookies are small text files placed on your computer, tablet or phone when you visit a web site. Types of cookie include:
